Information for Prospective Students
Applying for Services
A student requesting disability accommodations for the first time at the Colorado School of Mines must refer to Documentation Information and submit required documents, as well as a completed Request for Reasonable Accommodations form to Disability Services. To help ensure timely coordination of accommodations, submit documentation at least 60 days prior to the start of the semester.
Disability Services carefully reviews a student's documentation to determine whether the student is eligible to receive accommodations under the Americans with Disabilities Act Amendments Act (ADAAA) and/or Section 504 of the Rehabilitation Act. If additional information or clarification is needed, the student will be contacted by Disability Services. It is the student's responsibility to provide additional documentation if requested. Please allow a minimum of two weeks for review of your request.
The Transition from High School to College
Receiving Services
Once documentation is reviewed, the student is contacted by the Student Disability Services Coordinator. If eligible to receive accommodations, and the recommended accommodations are reasonable in the context of the academic program, the student is instructed to come into Disability Services to sign and date the Request for Accommodations and Release of Information forms. Procedures for implementing accommodations are discussed, and the student's rights and responsibilities are reviewed. The student is also apprised that individual circumstances may warrant modification of the accommodations initially granted. Original documentation as well as documentation of accommodations granted are kept on file in Disability Services, and the student receives a copy of the Confidential Letter of Required Accommodations sent to faculty each semester. If accommodations are needed in areas such as housing and facilities, appropriate offices are notified.
Note: To discuss the possibility of receiving accommodations not originally requested, the student must contact Disability Services. Additional documentation may be required.
Continuing Services
It is a student's responsibility to fill out the appropriate forms at the beginning of each school year (August) in order to arrange for accommodations for the fall and spring semesters. If accommodations are needed for field and/or summer sessions, it is up to the student to notify Disability Services by the end of the spring semester.
